Qualifications

• Minimum 4 years of nursing experience, with at least 3 years in acute care.
• Experience in critical care or emergency department preferred.
• Ability to coordinate transfers and document in EPIC.

Responsibilities

• Coordinate transfers to Froedtert facilities and partners.
• Collect clinical information from sending provider (diagnosis, tests, history).
• Determine level of care, transport mode, and location with accepting physician.
• Document transfer details in EPIC and collaborate with physicians and patient flow.
• Participate in conference calls and cross-train for nurse triage during off-hours.
