Waynesboro Family Medical Associates, LLP seeks an RN Care Coordinator to support patients with chronic conditions under supervision of two physicians and three nurse practitioners. The role involves health coaching, referrals, and coordination of care by phone and occasional visits.

The ideal candidate has an active PA RN license and strong communication skills, with experience in chronic care management and EMR systems. Flexible, patient-focused care environment with a family-oriented ethos.

Formación

• RN with PA license (current).
• Experience with chronic care coordination preferred.
• Comfort with patient education and referrals.
• Proficient with electronic health records and telecommunication.

Responsabilidades

• Provide Chronic Care Coordination under physician and NP supervision.
• Coordinate care plans, education, referrals, and coaching via phone and occasional visits.
• Assist transition of patients post-hospitalization to home or facility.
• Document care activities and coordinate with care team for condition changes.
