As part of a small team of clinicians, you will guide education, referrals, and transitions from hospitalization back to home, ensuring timely documentation and seamless care.

Qualifications

• RN with PA license and active licensure.
• Experience with chronic condition management preferred.
• Excellent interpersonal and communication skills.
• Ability to multi-task and manage time effectively.
• Proficiency with electronic medical records.

Responsibilities

• Provide Chronic Care Coordination via telephone and occasional visits.
• Coordinate health coaching, education, and referrals.
• Collaborate with physicians, nurse practitioners, patients, and families.
• Support transition of patients returning home after hospitalization.
• Document care plans and coordinate with Chronic Care Management program.
