Job summary

Valley Health Partners in Allentown, PA is seeking an LPN Care Manager to coordinate services within the Center for Women's Medicine Fairgrounds. The role supports care delivery, referrals, and comprehensive care management to high-risk patients, developing personalized care plans with a focus on chronic condition management and improved outcomes.

Required is an LPN license in PA with 3 years' clinical experience (MA or LPN) and EMR experience; BLS certification is needed.

Qualifications

• Three years of clinical MA or LPN experience in care management settings.
• Two years of prior EMR experience.
• Graduate of an accredited LPN program; PA LPN licensure.

Responsibilities

• Coordinate services within the health center and manage care plans.
• Participate in delivery of care and referrals for patients.
• Assess patient/family needs for high-risk patients and develop care plans.
• Collaborate with multidisciplinary care team to improve outcomes.
• Support chronic condition management and resource navigation.
