Job summary

Qualifications

• Two (2) years previous clinical experience.
• Three (3) years of experience in case management, social services or related field.
• Certified Case Manager.

Responsibilities

• Work with neonate, adolescent, adult, and geriatric patients.
• Determine appropriate setting of care and appropriateness of hospitalization.
• Provide support to address social drivers of health.

Skills

Excellent oral and written communication skills

Ability to interact with patients, families, physicians and hospital staff

Ability to type accurately at an approximate rate of 30 wpm

Education

Graduate of an accredited Registered Nursing School (RN)

Current License in the State of Georgia as a Registered Nurse or Nursing Compact
