Job summary

Northeast Georgia Health System is seeking a Registered Nurse to provide comprehensive assessment, planning, and coordination of patient care across units. You will collaborate with physicians, families, and the healthcare team to ensure efficient care transitions and length of stay management.

The role requires solid clinical skills in critical care, cardiac, medical, or surgical nursing, and strong communication. Weekend rotation and remote coverage may be required.

Qualifications

• Licensed to practice as an RN in Georgia.
• Associates Degree; graduate of an accredited school of nursing.
• 3–5 years of direct patient care and/or case management experience.
• Knowledge of reimbursement methodologies and regulatory requirements.

Responsibilities

• Monitor patients on assigned units and manage length of stay.
• Communicate with physicians, patients/families, and disciplines about discharge.
• Coordinate and facilitate patient care and interventions to prevent delays.
• Assess and coordinate post-acute discharge plans with care team.
• Ensure compliance with regulatory and DNV requirements.
• Lead discharge planning and coordinate community referrals and resources.
