Job summary

Northeast Georgia Health System is seeking an experienced Registered Nurse to provide comprehensive patient assessment, planning, and independent care coordination across units. You will collaborate with physicians, families, and the healthcare team to ensure quality outcomes and efficient care transitions for all ages from neonate to geriatric.

The role emphasizes length-of-stay management, regulatory compliance, and rounding with interdisciplinary teams, with cross-coverage and weekend

Qualifications

Licensed to practice as an RN in Georgia.
Graduate of an accredited school of nursing; Associate degree.
Three (3) to five (5) years of direct patient care and/or case management experience; knowledge of financial and discharge planning.

Responsibilities

Monitor patients on assigned units to ensure appropriate use of resources and manage length of stay.
Communicate with physicians, patients/families, and other disciplines about patient progress and discharge planning.
Coordinate and oversee patient care to ensure interventions occur in proper sequence and timeliness.
Identify and address potential delays in services; escalate to management when needed.
Assist with discharge planning and post-acute transitions; communicate plans with care team.
