Job summary

Glisan is seeking a qualified healthcare professional to coordinate resident care and documentation in a dynamic environment in Portland. You will communicate plans of care with residents and families, attend stand-ups, and support admission and discharge workflows.

The role requires nursing or social work-education, potential RN/LVN licensing, and 2+ years clinical experience with Medicare/Medicaid knowledge. Strong communication and organization are essential for success.

Qualifications

• Bachelor's Degree in Nursing or Social Work required or preferred.
• RN license preferred; LVN/LPN acceptable. Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.

Responsibilities

• Communicate regularly with residents and their families about Plans of Care, PT, OT and other treatment protocols.
• Attend and participate in morning meetings to facilitate team communications.
• Answer residents' questions about care, treatment plans, illness progression and other issues.
• Monitor and adjust residents' statuses based on changing needs.
• Organize and prioritize daily work by assessing residents' needs in area of responsibility; complete documentation as required.
• Perform utilization review activities to provide appropriate, timely and cost-effective care.
• Coordinate care with residents, care providers, facilities, financial services, and third-party payers.
• Oversee admissions and discharge activities; coordinate referrals to and from the facility.
