Corewell Health is seeking a full-time RN Care Manager at Pennock Hospital in Hastings, MI. The role focuses on planning and prioritizing care for individuals and populations, coordinating services, and improving health outcomes within a managed care environment.

The position requires an RN license in Michigan, BSN within 7 years for new hires, and 2 years of relevant clinical or case management experience; 5 years preferred. Work is day shift, 40 hours, with some weekend coverage as described.

Qualifications

• BSN degree (or equivalent) must be obtained within 7 years for RN role at Corewell Health Hospitals
• 2 years of relevant experience in clinical nursing and/or case management or managed care
• 5 years of relevant experience preferred
• Registered Nurse (RN) license for Michigan upon hire

Responsibilities

• Assess internal and external referrals to identify patient needs, resources, and benefits
• Develop care plans and coordinate with PCPs and other clinicians for continuity of care
• Educate patients, families and care team; coordinate education referrals as needed
• Communicate with patients, providers, and payers to coordinate services across the continuum
• Measure patient care outcomes and analyze data trends for groups
• Document patient data, plans, interventions and outcomes per department guidelines
