Job summary

Virginia Commonwealth University Health in Richmond, VA is seeking an RN Care Coordinator for the Cardiology/Cardiothoracic Surgery ICU. This role integrates with physicians, social workers, discharge planners and payers to guide patients through the acute episode and across the continuum.

The successful candidate will hold a Virginia RN license, have at least three years of acute care experience, and preferably a master's in nursing or related field.

Qualifications

• Licensed RN in Virginia or eligible to practice.
• Current Virginia RN licensure required.
• BSN from accredited nursing program or Bachelor’s in biological, health or health sciences.
• Minimum 3 years of acute care nursing experience.
• Master’s degree in nursing or related field preferred.
• Experience in utilization management / care coordination preferred.
• Strong teamwork and communication with multidisciplinary care teams.

Responsibilities

• Collaborate with physicians, social workers and discharge planners to coordinate care.
• Facilitate transitions from hospital to home or other care settings.
• Work with payers to ensure appropriate reimbursement and utilization.
• Support CMS guidelines and QIO quality initiatives.
• Maintain professional rapport with providers, patients and staff.
