Job summary

VCU Health is seeking an RN Care Coordinator to join its multidisciplinary team in Virginia. The role guides patient progress from the acute episode through post-discharge with a focus on quality, safety, and cost-effectiveness.

Requirements include Virginia RN licensure or eligibility, a BSN or equivalent degree, and a minimum of three years in acute care. Evening/day shifts are available, with strong collaboration across care teams.

Qualifications

• Licensed Registered Nurse in the State of Virginia or eligible.
• Complete 15 Continuing Education Units per year.

Responsibilities

• Works with attending and consulting physicians to facilitate effective transition through hospitalization.
• Collaborates with all members of the multi-disciplinary team to coordinate patient care and ensure appropriate reimbursement.
• Seeks social workers to resolve psychosocial issues and develop complex transition/discharge plans.
• Acts as liaison between hospital and community agencies for exchange of information.
• Maintains compliance with QIO and CMS guidelines.
• Uses medical necessity criteria to assess level and setting of care and assists with denial/appeals.
• Participates in patient safety initiatives and aligns with VCUHS Mission and Values.
• Maintains professional rapport with providers, patients/families, and internal customers.

Education

Baccalaureate Degree in Nursing

Bachelor’s degree in biology/health/social sciences

Master’s Degree in Nursing or related field
