Sign-On Bonus!!! We are offering qualified and experienced candidates a sign-on bonus not to exceed 10% of salary for this position.

The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for identified patients requiring these services. The role involves ensuring the individual's health needs are met while promoting quality and cost-effective outcomes, following hospital policies, standards of practice, and Federal and State regulations. The position emphasizes care coordination, communication, and collaboration with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute service providers to optimize outcomes at the appropriate level of care. The RN Care Coordinator advocates for patients and families by identifying and addressing patient choices, spiritual needs, cultural, language, and socioeconomic barriers to care transitions. Additionally, the RN Care Coordinator aims to enhance the patient experience.

Qualifications

• Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing in lieu of 1 year experience.
• California RN license
• AHA BLS card
• LA City Fire Card required within 90 days of hire.
• Ability to pass annual Inter-rater reliability test for Utilization Review products used.
• Ability to apply clinical guidelines to ensure progression of care.
• Critical thinking and problem-solving skills.
• Effective collaboration with multiple stakeholders.
• Professional communication skills.
• Understanding of how utilization management and case management programs integrate.
• Ability to work as a team player and assist other team members as needed.

Preferred

• Graduate of an accredited nursing school (Bachelor's Degree in Nursing - BSN) or related healthcare field.
• At least five (5) years of nursing experience.
• Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred.
• Knowledge of managed care and payer environment preferred.
