Job Description

Job Title: Registered Nurse Well Care Coordinator

Location: Remote (Evansville, Indiana) Must be an Indiana Resident (field visits required)

Hours: Monday - Friday 8am-5pm

Pay: $48 hourly | Weekly pay

Travel Requirement: 25% - 50%

Job Summary: The Community Well Care Coordinator oversees care coordination and complex case management for PathWays to Aging members in the community, not receiving HCBS or NFLOC. Must be a registered nurse or similar with extensive experience in care coordination for members aged 60+. Works under the Health Services Director to manage care teams, develop and implement care plans, and coordinate physical and behavioral health services. Collaborates with various stakeholders and participates in state-level meetings.

Primary Responsibilities:

1. Develop and support staff, set performance goals, and ensure accountability.
2. Understand contractual requirements to meet members' needs.
3. Identify and lead process improvement projects.
4. Ensure workflow standardization, conduct audits, and ensure compliance.
5. Assess, plan, and implement individualized care strategies.
6. Facilitate referrals to social services and coordinate care.
7. Communicate health information to stakeholders to ensure quality care.
8. Advocate for patients and families.
9. Participate in staff training and coaching.
10. Conduct bi-annual field visits with team members.
11. Collaborate with internal and external stakeholders, including medical directors and state staff.

Required Qualifications:

• Resident of Indiana
• BSN or equivalent experience
• Unrestricted RN license in Indiana
• Community health care experience
• Knowledge of Indiana Medicaid, Medicare, long-term care
• Staff coaching or mentoring experience
• Proficiency with Microsoft Word and Windows environment

Preferred Qualifications:

• 3+ years in healthcare case management
• Background in managed care
• Certified Case Manager (CCM)
• Experience with long-term social supports, utilization review, risk management
