In addition to primary care, Bluestone has a highly developed care coordination model for over 14,000 seniors and individuals with disabilities in Minnesota. Bluestone Care Coordination partners with Minnesota health plans to support members receiving medical assistance through SNBC & MSHO programs. Care Coordinators, who are registered nurses or licensed social workers, work directly with members to assess their physical, mental, and social needs, facilitating services and communication across care teams to support their best interests and close care gaps.

Position Overview:

Join our team as a Care Coordinator, working with the SNBC population (ages 18-64) in the community and group homes. This role is primarily remote, with regular travel within your local area to serve members and the community.

Schedule: Full-time, day shifts, no evenings, weekends, or holidays. Hours are 8 am to 5 pm Monday through Thursday, and 8 am to 3 pm on Fridays.

Location: Forest Lake and surrounding Hennepin County areas.

Salary Range: $65,000 - $75,000

Responsibilities:

1. Managing member caseloads within your geographic area.
2. Coordinating face-to-face visits.
3. Conducting annual assessments, including PCA assessments and customized living tools.
4. Reviewing health needs, setting goals, and developing care plans.
5. Connecting members with community resources and services.
6. Completing documentation.
7. Collaborating with medical care teams to meet quality measures and utilize management tools.
8. Supporting members during care transitions and ensuring safe discharges with follow-up plans.

Qualifications:

• Current Minnesota Registered Nurse license or ORMN Licensed Social Worker (new grads encouraged).
• One or more years of experience with geriatric or disabled populations in case management or care coordination roles preferred.
• Valid driver’s license.
