Job summary

The role requires RN licensure in California, 3+ years in care coordination or nursing leadership, and CCM/ACM certification within one year. Sign-on bonus available for eligible hires.

Qualifications

• Bachelor's Degree in Nursing (BSN) or Masters in Social Work (MSW) or equivalent education and experience
• Minimum of three (3) years of clinical care coordination experience or three (3) years of acute nursing leadership experience with care coordination knowledge
• Operational experience or staff development in healthcare environment
• Current RN license in California (no license required for MSW)
• CCM or ACM certification required within 1 year of hire
• Knowledge of clinical guidelines, effective communication, analytical thinking, and care coordination processes
• Experience with CMS standards and responsibilities

Responsibilities

• Provide operational leadership for the Care Coordination department through supervision and performance management
• Coordinate and implement care coordination programs and workflows to support patient progression and outcomes
• Manage daily department operations including staffing, scheduling, productivity, and resource allocation
• Supervise front-line care coordination staff and ensure accountability for performance and service quality
• Ensure timely completion of assessments, discharge planning, utilization management collaboration, documentation, and care transitions
