Job summary

Allina Health is seeking a 1.0 FTE Care Manager to provide care management support for specialty patients in Minnesota. Position involves 8-hour day shifts, no weekends, and occasional travel to support the department.

Responsibilities include collaborating with care teams, coordinating plans of care, and documenting outcomes in the EMR. Requires RN licensure in MN and 2+ years of relevant nursing experience.

Qualifications

• Bachelor's degree in nursing
• 2+ years of nursing experience in area of clinical specialty, care coordination or care management
• Current BLS certification from the American Heart Association and Red Cross
• RN license in MN; WI license if serving WI patients by phone/virtual visits

Responsibilities

• Provides care management support to a panel of patients requiring specialty services.
• Interacts with and supports specialty and primary care providers and the care team across the continuum.
• Assesses, plans, coordinates, monitors and updates the plan of care with evidence-based practices.
• Leads quality improvement initiatives and program development.
• May travel to multiple locations to support patients and department.
