Wellstar Kennestone Regional Medical Center is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure effective discharge planning and progression of care. You will collaborate with physicians, nurses, social workers, and case managers to optimize outcomes and resource use.

Qualifikationen

• Excellent written and verbal communication skills.
• Strong assessment, interview, organizational and problem-solving abilities.
• Knowledge of local, state and federal regulations pertaining to discharge planning.
• Ability to work collaboratively with physicians, care team and patient/family.

Aufgaben

• Assessment: initiate assessment of chronic disease management needs and psychosocial factors; coordinate with PAS, financial counselor and UM nurse for insurance needs.
• Disposition Planning: manage discharge planning and develop individualized plans with patients and families; identify barriers.
• Care Progression: facilitate communication among physicians and care team to ensure timely progression and appropriate care level.
• Documentation: maintain clear and up-to-date medical records and progress notes; track services and avoidable days.
• Professional Development: complete mandatory education and support department goals; mentor students if applicable.
