Job summary

The Valley Hospital in Paramus is seeking a Clinical Care Coordinator, Case Manager to coordinate patient progression and transitions of care across the continuum. You will collaborate with patients, families, physicians, payers, HORC, and the interdisciplinary team to promote resource utilization, timely placement, and optimal outcomes.

The role requires a BSN, RN licensure in New Jersey, and 5 years of clinical nursing experience with preference for case management and utilization review.

Qualifications

• Bachelor's degree in nursing (BSN) required.
• Active RN license in New Jersey.
• ANCC Case Management Certification preferred.
• Minimum five years of clinical nursing experience required.
• Experience in medical, surgical, perioperative, procedural, or ED nursing preferred.
• Experience in case management and utilization review preferred.

Responsibilities

• Coordinate patient progression and transitions of care across the continuum.
• Review admission status, discharge planning, and utilization management.
• Collaborate with patients, families, physicians, payers and HORC.
• Promote appropriate resource utilization and regulatory compliance.
