Valley Health System is seeking a Clinical Care Coordinator, Case Manager to coordinate patient progression and transitions of care across the continuum. The role collaborates with patients, families, physicians, payers, HORC, and interdisciplinary teams to promote resource utilization and efficient throughput.

The position may involve inpatient, surgical, or ED operations. Required is a BSN and RN licensure in New Jersey, with 5+ years of clinical nursing; ANCC Case Management Certification

Qualifications

• Bachelor's degree (BSN) in nursing required.
• Registered Nurse.
• Licensed in State of New Jersey.
• ANCC Case Management Certification preferred.
• Experience Minimum five (5) years of clinical nursing experience required.
• Experience in medical or surgical, perioperative, procedural, or emergency nursing preferred.
• Experience in case management and/or utilization review strongly preferred.

Responsibilities

• Coordinate patient progression and transitions of care across the continuum.
• Oversee utilization management and discharge planning.
• Review admission status and promote appropriate resource utilization.
• Collaborate with patients, families, physicians, payers, HORC, and interdisciplinary teams.

Education

Bachelor's degree (BSN) in nursing

Registered Nurse

Licensed in State of New Jersey

ANCC Case Management Certification

Experience in clinical nursing (5+ years)

Experience in case management and/or utilization review
