Valley Health System (New Jersey) is seeking a Clinical Care Coordinator, Case Manager to oversee patient progression across the continuum. The role partners with patients, families, physicians, payers, HORC, and teams to promote appropriate resource utilization and timely placement.

The ideal candidate holds a BSN, is an RN licensed in NJ, and has 5+ years of clinical nursing experience with a preference for case management or utilization review. Knowledge of CMS and MCG guidelines is required.

Qualifikationen

• Bachelor's degree in nursing (BSN) is required.
• Registered Nurse licensed in the State of New Jersey.
• ANCC Case Management Certification preferred.
• Minimum five years of clinical nursing experience.
• Experience in case management and/or utilization review preferred.
• Knowledge of CMS regulations, Medicare/Medicaid, and managed care requirements.

• Coordinate patient progression, utilization management, discharge planning, and transitions of care.
• Review admission status and ensure appropriate resource utilization.
