Job summary

Empath Health is seeking a Registered Nurse to join Tidewell Hospice’s southwest Florida team, providing compassionate end-of-life care across multiple facilities. The RN will conduct comprehensive assessments, develop and manage Plans of Care, and collaborate with the IDT and partnering facilities to ensure patient-centered outcomes.

You will administer and document medications, educate patients and families, and adapt teaching to individual needs while working in a supportive, mission-driven

Qualifications

• Must have an active Florida RN license.
• Associate degree in Nursing or equivalent required; hospital or hospice experience preferred.
• Experience with terminally ill patients or life-threatening illnesses is a plus.

Responsibilities

• Obtain patient history and perform age-specific assessments.
• Develop and update Plan of Care with patient and family.
• Coordinate care with IDT and partnering facilities under policy guidelines.

Job description

Schedule: 8am -8:30pm, Candidates will assist at Lakewood Ranch, Sarasota, Bradenton, and Venice Hospice houses.

What you’ll Do

• Supports and carries out the organization’s mission, vision, and values on a consistent basis.
• Demonstrates the ability to obtain patient history, perform an age‑specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
• The RN will identify patient/family goals, develop a Plan of Care with patient, family, IDT, attending physician and/or partnering facility and continually evaluate progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change (RN performs as case manager).
• The RN will demonstrate the ability to administer, monitor and document medications via all routes using age‑specific care practices and provide appropriate patient/family teaching
• The RN will identify age‑specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow‑through to match.

What You’ll Need

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two‑year college or technical school. Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting. An equivalent of education and experience may be considered.

Licensure:

• Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
