Job summary

Community Care is hiring an RN Case Manager to join our Family Care Program team in Waukesha. This full-time role emphasizes team collaboration, community case management, and helping elderly and disabled adults live independently in the community.

The position includes a $10,000 sign-on bonus for external hires and requires a Wisconsin RN license, a valid driver's license, and at least 2 years of relevant nursing experience. Hours are Monday–Friday, 8:00 AM–4:30 PM.

Qualifications

• Associates degree in Nursing required.
• Bachelor’s degree preferred.
• Wisconsin RN license required.
• Eligibility for long-term care screener certification required.
• WI driver’s license required.
• 2 years of recent experience with older adults or disabilities in a nursing role required.
• Community nursing experience preferred.

Responsibilities

• Performs discipline-specific initial and ongoing assessments, conducts home visits, and collaborates with families and supports for information.
• Develops and maintains care plans focused on long-term needs, outcomes, and supports; ensures service coordination and quality.
• Monitors services, follows up on outcomes, and adjusts plans to achieve patient goals.

RN Case Manager Job Responsibilities:

• Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
• Develops and maintains care plans, including members’ long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.

RN Case Manager Job Requirements:

• Education: Associates degree in Nursing Required, Bachelors Degree preferred.
• Licensure: Wisconsin Licensed Registered Nurse
• Licensure: Eligibility for long-term care functional screener certification is required
• Licensure: WI Drivers License
• Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required
• Preferred 2 years of work in community nursing

RN Case Manager Job Schedule:

Monday- Friday 8:00 AM-4:30 PM.
