Job summary

Diversicare is seeking a dedicated Care Assessment Coordinator to join our team and help ensure accurate MDS assessments and compliant care plans for residents. You will lead the RAI process, coordinate interdisciplinary input, and monitor regulatory guidelines across state and federal requirements.

Responsibilities include managing MDS timelines, facilitating care coordination meetings, and communicating with patients, families, and payers.

Qualifications

• Must hold current RN license in state of employment.
• LPNs will be considered in centers where an RN is present.
• Experience with MDS/RAI processes and regulatory guidelines preferred.
• Knowledge of PDPM, managed care reimbursement, utilization and case management is preferred.
• Strong leadership and project management skills; excellent communication.

Responsibilities

• Maintain and improve compliance with state and federal MDS requirements.
• Coordinate the RAI process with timely MDS assessments and accurate documentation.
• Monitor Quality Measures with the interdisciplinary team to ensure accurate MDSs.
• Ensure Medicare/Medicaid regulatory guidelines are completed accurately and timely.
• Lead care planning and discharge planning processes with timely communication to patients/families.
