Job summary

Elara Caring is hiring a Registered Nurse – Behavioral Health for the Hartford area. The role focuses on delivering high-quality in-home behavioral health visits, managing a caseload, and coordinating care plans with physicians and the care team.

You will educate families, supervise aides, and participate in end-of-life support where needed. This is a full-time days position with a strong benefits package.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians and care team.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Supervise and support BH Aides and LPNs while ensuring quality care.
