Job summary

Elara Caring seeks a Registered Nurse Behavioral Health to serve the Hartford and Glastonbury areas. This full-time position offers consistent daytime hours and a flexible approach to care, emphasizing high-quality behavioral health support in patients’ homes.

You will assess and coordinate care, develop individualized plans, and educate patients and families while collaborating with physicians and the care team.

Qualifications

• Active RN license in the state.
• Graduate of an accredited nursing program (ADN/BSN).
• Minimum 1+ year nursing experience in a clinical setting.
• Home health experience preferred but not required.

Responsibilities

• Assess patients’ physical, emotional, social, and environmental needs.
• Manage a caseload and update individualized care plans.
• Complete initial assessments and ongoing evaluations during visits.
• Monitor patient progress and communicate changes with the care team.
• Educate patients and families about the care plan and behavioral health support.
• Supervise and support Behavioral Health Aides and LPNs.
