Job summary

Elara Caring is seeking a Registered Nurse for Behavioral Health visits in Middletown with full-time hours and comprehensive benefits. The role focuses on keeping clients at home while delivering high-quality care and coordinating plans with the care team.

The ideal candidate has an active RN license, 1+ year of clinical nursing experience, and may have Home Health experience. A valid driver’s license and ability to lift up to 50 pounds are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
