Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health for full-time day visits in the Hartford area. You will assess, coordinate, and update care plans, delivering compassionate behavioral health support in clients’ homes.

The role requires an active RN license, 1+ year of nursing experience, and flexibility for home-based visits. Travel between patient homes is required; driver’s license or reliable transportation is needed. Competitive benefits and total compensation are offered.

Qualifications

Current State license as a Registered Nurse.
Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
1+ year of nursing experience in a clinical care setting.

Responsibilities

Assess patients' physical, emotional, social, and environmental needs.
Manage a caseload and develop, coordinate, and update individualized care plans.
Complete initial assessments and provide ongoing evaluations during patient visits.
Monitor patient progress and communicate changes with physicians and the care team.
Educate patients, families, and caregivers about the plan of care and behavioral health support.
Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
