Heritage Ridge Senior Living at Johnstown is seeking a Registered Nurse Assessment Coordinator (RNAC). This role focuses on determining the appropriate level of care for residents and supervising all MDS assessments to ensure timely completion, while participating in pre-admission evaluations.

The RNAC will follow Resident Care Plans, maintain precise documentation, and contribute to the Quality Assurance Program. Active RN licensure and prior long-term care or RNAC experience are preferred.

Qualifications

• Possess an active and good-standing RN License.
• Long term care experience preferred.
• Experience as an RNAC preferred.

Responsibilities

• Determine appropriate level of care for residents.
• Schedule and oversee all resident MDS Assessments.
• Participate in pre-admission process to assess level of care.
• Review MDS assessments and related forms for timely completion.
• Participate in the Quality Assessment and Assurance Program.
• Understand and follow Resident Care Plans.
• Ensure accurate documentation during shifts.
• Complete charting, reporting, and progress notes.
