Job summary

Twin Lakes Rehabilitation and Healthcare Center in Greensburg, PA is seeking a full-time Registered Nurse Assessment Coordinator (RNAC) to lead our nursing team. The RNAC will conduct resident assessments, oversee the RAI process, and support quality initiatives while collaborating with the interdisciplinary team.

Candidates must be an RN with long-term care assessment experience, possess PA CMI and Medicare billing knowledge, and complete routine screenings and background checks.

Qualifications

• RN with assessment skills in a long-term care setting.
• Knowledge of state and federal regulations.
• Associate's in Nursing required; BSN preferred.
• At least 1 year RNAC experience in a skilled nursing facility with MDS.
• Proficiency in PA CMI and Medicare billing knowledge.
• Knowledge of PPS (PDPM) and managed care/insurance reviews.

Responsibilities

• Conduct and coordinate resident assessments per state regulations and facility goals.
• Oversee the RAI process following Medicare/Medicaid/OBRA guidelines.
• Develop, implement, and maintain a QA program for resident assessment.
• Participate in facility surveys by government agencies.
• Monitor QI/QM reports and drive corrective actions.
• Provide education to the interdisciplinary team involved in the RAI process.

Responsibilities of the Registered Nurse Assessment Coordinator (RNAC):

• Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
• Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
• Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
• Participate in Facility surveys (inspections) made by authorized government agencies.
• Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
• Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.

Requirements of the Registered Nurse Assessment Coordinator (RNAC):

• Registered Nurse (RN) with assessment skills in a long-term care setting
• Must have knowledge of state and federal regulations
• Associate's in Nursing Required, BSN preferred.
• Must have at least 1 year experience as an RNAC in a skilled nursing facility completing MDS
• Proficient in PA CMI
• Registered Nurse Assessment Coordinator (RNAC) must have Medicare billing knowledge and experience.
• Knowledge of PPS (PDPM)
• Experienced with Managed Care and Insurance Reviews
• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
