Heritage Ridge Senior Living at Johnstown is seeking a compassionate Registered Nurse Assessment Coordinator (RNAC) to determine the appropriate level of care for residents and oversee MDS assessments. The RNAC will participate in the pre-admission process and ensure timely completion of forms.

Responsibilities include documenting accurately, reviewing care plans, and contributing to the Quality Assessment and Assurance program within a long-term care environment.

Qualifications

• Active RN license in good standing.
• Experience coordinating MDS assessments.
• Long-term care experience preferred.

RESPONSIBILITIES:

• As aRegistered Nurse Assessment Coordinator (RNAC), your primary responsibility is determining the appropriate level of care for our residents.
• Scheduling and overseeing all resident MDS Assessments.
• Participating in the pre-admission process to determine potential residents’ level of care.
• Reviewing all MDS assessments and related forms to certify timely completion.
• Participate in the Quality Assessment and Assurance Program.
• Understand and follow Resident Care Plans.
• Ensure accurate documentation during your shift.
• Complete accurate charting, reporting, and progress notes.

REQUIREMENTS:

• Possess an active and good-standing RN License.
• Long term care experience preferred.
• Experience as an RNAC preferred.
