Serenity Spring Senior Living at Green Bay is seeking a compassionate Full-Time Registered Nurse Assessment Coordinator (RNAC) to join our healthcare team. The RNAC will assess, document patient needs, coordinate care plans, and ensure regulatory compliance to improve patient outcomes.

The role requires an active Wisconsin RN license, at least 1 year in long-term care, strong MDS knowledge, and excellent communication.

Qualifications

• Current RN license in Wisconsin.
• Minimum of one year of experience in long-term care or similar healthcare settings.
• Strong knowledge of MDS assessments and Medicare/Medicaid regulations.
• Excellent communication and interpersonal skills.
• Detail-oriented with a commitment to accuracy and compliance.

• Proficient computer skills and familiarity with EHR systems.

Responsibilities

• Conduct comprehensive patient assessments to determine care needs.
• Collaborate with healthcare professionals to develop and update individualized care plans.
• Ensure compliance with Medicare and Medicaid regulations and guidelines.
• Accurately complete and submit MDS assessments.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to clinical staff regarding assessment and documentation best practices.
• Participate in interdisciplinary team meetings to discuss patient care and coordination.
• Maintain accurate and up-to-date patient records.
