Job summary

Heritage Ridge Senior Living at Johnstown is seeking a Registered Nurse Assessment Coordinator (RNAC) to determine the appropriate level of care for residents and to oversee MDS assessments. This role includes participation in pre-admission evaluations and ensuring timely completion of forms and care plans.

You will document accurately, report progress, and contribute to the Quality Assessment and Assurance Program while pursuing professional growth within our organization.

Qualifications

• Active RN license in good standing.
• Long-term care experience preferred.
• Experience as an RNAC preferred.

Responsibilities

• As a Registered Nurse Assessment Coordinator (RNAC), your primary responsibility is determining the appropriate level of care for our residents.
• Scheduling and overseeing all resident MDS Assessments.
• Participating in the pre-admission process to determine potential residents’ level of care.
• Reviewing all MDS assessments and related forms to certify timely completion.
• Participate in the Quality Assessment and Assurance Program.
• Understand and follow Resident Care Plans.
• Ensure accurate documentation during your shift.
• Complete accurate charting, reporting, and progress notes.
