Job summary

St. Anne Home in Greensburg, PA is hiring a Registered Nurse Assessment Coordinator (RNAC) to ensure accurate RAI and Care Management from admission to discharge.

This full-time on-site role requires PA RN licensure and experience in MDS, long-term care, and care plan management. Responsibilities include completing all MDS sections, leading Care Plan meetings, ensuring documentation supports MDS coding, and collaborating with the IDT to plan resident care.

Qualifications

• RN licensure in Pennsylvania.
• Minimum of 1 year of MDS experience.
• Minimum of 1 year of Long-Term Care experience.
• Minimum of 1 year of Care Plan experience.

Responsibilities

• Complete all required sections of the MDS.
• Lead and document Care Plan meetings and monitor progress.
• Ensure documentation supports MDS coding in the medical record.
• Collaborate with the IDT to assess needs and plan resident care.
• Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
