Serenity Spring Senior Living at Northwood in Jasper, Indiana is seeking an experienced RN Assessment Coordinator (RNAC) to ensure residents receive appropriate care. The role focuses on completing the Resident Assessment Instrument and coordinating services with physicians, families, and payers to support resident well-being.

The ideal candidate has an active Indiana RN license, 3–5 years of nursing home experience, and prior RNAC/MDS coordination experience.

Qualifications

• Responsible for completion of the Resident Assessment Instrument in accordance with federal and state regulations and company policy and procedures.
• Acts as in-house case manager by considering all aspects of the resident's care and coordinating services with physicians, families, third party payers and facility staff.
• Current IV certification preferred
• Registered Nurse with current active Indiana license
• Minimum three to five years(3-5) years of clinical experience in Nursing Home
• Prior experience as an RNAC/MDS coordinator

Responsibilities

• Complete the Resident Assessment Instrument according to regs and policy.
• Coordinate services with physicians, families, payers, and staff as in-house case manager.
