Job summary

West Park is seeking a knowledgeable Registered Nurse Assessment Coordinator (RNAC) to lead and coordinate MDS assessments, ensuring timely completion and regulatory compliance for residents.

The RNAC will support quality outcomes, collaborate with an interdisciplinary team, and provide staff training on assessment practices and documentation standards. State RN license, MDS 3.0 experience, and PDPM knowledge are required or highly preferred.

Qualifications

• Active RN license in the state where practicing
• Experience with MDS 3.0 and care planning required
• Knowledge of Medicare/Medicaid regulations, PDPM and reimbursement guidelines
• Detail-oriented with strong organizational, clinical, and analytical skills
• Experience in long-term care or skilled nursing preferred
• AANAC certification a plus or willingness to obtain

Responsibilities

• Coordinate and complete MDS assessments and related documentation within regulatory timelines
• Ensure accurate coding reflect resident care and maximize appropriate reimbursement
• Collaborate with interdisciplinary team on care plans and case reviews
• Monitor quality indicators and assist with QAPI efforts
• Stay current with CMS guidelines, state regulations, and company policies
• Provide staff training and support on assessment practices and documentation standards
• Participate in audits, surveys, and performance reviews to maintain compliance
