Job summary

West Park is seeking a knowledgeable and compassionate Registered Nurse Assessment Coordinator (RNAC) to join our team. This leadership role coordinates and oversees MDS assessments, ensuring regulatory compliance and supports quality outcomes for residents.

The RNAC will shape care plans, collaborate with an interdisciplinary team, and provide training on assessment practices. A current RN license and MDS 3.0 experience are required; AANAC certification is a plus.

Qualifications

  • Active RN license in the state of practice.
  • Experience with MDS 3.0 and care planning.
  • Knowledge of Medicare/Medicaid regulations, PDPM, and reimbursement guidelines.
  • Detail-oriented with strong organizational, clinical, and analytical skills.
  • Experience in long-term care or skilled nursing preferred.
  • AANAC certification a plus (or willingness to obtain).

Responsibilities

  • Coordinate and complete the Minimum Data Set (MDS) assessments and related documentation in accordance with regulatory timelines.
  • Ensure accurate coding to reflect resident care and maximize appropriate reimbursement.
  • Collaborate with interdisciplinary team members on care plans and case reviews.
  • Monitor quality indicators and assist with QAPI (Quality Assurance and Performance Improvement) efforts.
  • Stay current with CMS guidelines, state regulations, and company policies.
  • Provide staff training and support on assessment practices and documentation standards.
  • Participate in audits, surveys, and performance reviews to maintain compliance.
