Community Medical Centers is seeking a skilled RN at the CMC Respite Center in Stockton, CA. You will provide skilled nursing care within scope, coordinate care with MDs, PAs, and NPs, and ensure patient safety and quality care. This full-time role requires CA RN licensure and STuexperience with EHRs.

You will promote best practices, participate in care coordination, and support continuous improvement across the clinic team while traveling between sites as needed.

Qualificações

• Licensed in the State of California as a Registered Nurse.
• IV certification preferred.
• Proficient with Microsoft Office applications.
• Proficient using an Electronic Health Records system; Next Gen EHR preferred.
• Ability to travel between sites; ambulatory care experience preferred.
• Current Basic Life Support certification.

Responsabilidades

• Manages chronic disease in collaboration with the team and motivates patient self-management.
• Provides transition care management for discharged patients.
• Performs triage and emergency care as needed; supervises staff as required.
