Job summary

Heart to Heart Hospice in Jasper, IN, seeks an experienced Registered Nurse for full-time home health visits. You will assess and treat patients at home, develop care plans with physicians, and ensure timely documentation and communication with the care team.

Two years of RN experience is preferred. A valid RN license and driver's license with reliable transportation are required; a sign-on bonus of $5,000 is offered along with competitive pay, PTO, holidays, and 401k matching.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and valid auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including death pronouncement and crisis management.
• Develop and revise nursing care plans with the physician and IDG.
• Use excellent customer care with internal/external customers, including proper phone etiquette.
• Prepare clinical notes and progress updates showing progress toward goals.
• Inform physician and staff of changes in patient needs and intervention outcomes.
• Timely submission of all documentation.
• Perform other duties as required and ensure compliance with laws and policies.

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
