Job summary

Heart to Heart Hospice in Alaska is seeking a dedicated Registered Nurse (RN) to join our team and provide compassionate care to patients with life-limiting illnesses. You will observe and document patient conditions, implement care plans, and coordinate with physicians and families to ensure quality outcomes.

Active state RN license and a valid driver's license are required; two years of RN experience in healthcare/clinical settings is preferred.

Qualifications

  • State RN license required.
  • Valid driver's license and reliable transportation.
  • Two years RN experience preferred in healthcare/clinical setting.

Responsibilities

  • Observe, assess, document and report resident conditions during rounds.
  • Implement the individualized Plan Of Care as directed.
  • Identify significant changes in resident condition and act per policies.
  • Oversee and monitor Certified Nurse Aides.
  • Discharge residents per facility.
  • Complete Nursing Assessments for Physician Calls prior to contacting Physician on changes.
  • Communicate with the resident’s Point of Contact for status updates or changes.
  • Provide training to other staff as needed.
  • Maintain excellent customer care and proper phone etiquette with internal and external customers.
  • Submit all documentation timely.
  • Perform other duties as required.
  • Ensure compliance with laws, Medicare regulations, and policies.

Hospice House Staff Nurse - RN
What You Must Have:
  • Current State Registered Nurse License
  • Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
  • Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:
  • During rounds observes, assess, documents and reports resident conditions and/or changes
  • Implement the individualized Plan Of Care as directed
  • Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures
