Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to provide compassionate home visits in Terre Haute, IN. The role includes patient assessment, care plan development, crisis management, and coordination with physicians to ensure quality end-of-life care.

The on-call schedule is 7 on / 7 off with full-time benefits, including PTO, paid holidays, 401k matching, and a company car for qualifying individuals. Mobility and driving are essential.

Qualifications

• Graduate from an accredited Diploma, Associate or Baccalaureate Nursing program.
• Current State RN License.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment, and support.
• Develop and revise comprehensive nursing care plans with physician and IDG.
• Maintain excellent customer care with internal and external customers.
• Prepare progress notes showing progress toward goals.
• Inform physicians of changes and outcomes; monitor patient/family response.
• Ensure timely documentation and compliance with laws and policies.

What You Must Have:

• Graduate from an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
