Job summary

Interim HealthCare of the Twin Cities in Shoreview, MN is seeking an on-call RN to triage and visit patients in their homes after hours. The role follows a 7 days on/7 days off schedule, with on-call hours typically from 5pm to 8am on weekdays and 8am to 8am on weekends.

Required are MN RN licensure, active license in good standing, and CPR certification. Minimum 12 months RN experience in the last five years, with hospice/home care experience preferred, and a valid driver’s license.

Qualifications

• Licensed RN in MN with active license in good standing.
• Minimum 12 months RN experience within the last 5 years (or evidence of current clinical knowledge).
• Hospice or home care experience preferred.
• Valid driver’s license and reliable transportation.
• CPR certification required.

Responsibilities

• Be available to take calls and triage patients after normal business hours.
• Provide in-home visits for patients as needed.
• Operate on a full-time block schedule (7 days on/7 days off).
• Triages and coordinates care for patients remotely during after-hours periods.
