Park Nicollet is recruiting a Registered Nurse to join the Advanced Care Primary Care Clinic team. The RN will coordinate care for patients with complex chronic illnesses, collaborating with Pharmacy, Nursing, and Social Work to provide wrap-around services.

Experience with care coordination, EPIC proficiency, and strong teamwork are essential for success in this role and improving patient outcomes.

Qualificações

Graduated from an accredited school of nursing.
Associate degree or diploma required.
Maintain a current Minnesota registered nurse licensure.
BLS provider status per AHA guidelines; no online-only courses.
Proficiency with EPIC.
Strong interdisciplinary communication and collaboration.
Ability to work effectively in a team-oriented environment.

Responsabilidades

Provide comprehensive nursing care to patients with complex chronic illnesses.
Coordinate patient care with the multidisciplinary team.
Utilize EPIC to document patient information and care plans.
Follow established protocols to ensure high-quality patient care.
Engage in interdisciplinary communication and collaboration to optimize outcomes.
Educate patients and families about managing chronic illnesses and resources.
