Park Nicollet Health Services is seeking a Registered Nurse to join the Burnsville Primary Care team. This role involves triage, direct patient care, and collaboration with clinicians to improve patient outcomes in a diverse patient population.

The RN will work at the top of their license, performing assessments, immunizations, and care coordination while maintaining patient confidentiality and following professional standards and clinic protocols.

Formación

• Current Minnesota RN licensure.
• BLS provider status per AHA guidelines.
• No online-only courses accepted.

Responsabilidades

• Provide triage for patient calls and walk-in needs.
• Deliver direct patient care including assessments and injections.
• Identify and address care gaps between visits.
• Collaborate with providers and clinic team members.
• Provide culturally responsive patient education and follow-up.
• Document care accurately in the EHR.
