Job summary

The Cleveland Clinic is seeking a Nursing Admission Specialist to initiate and resume home care for diverse patients across the western Cleveland area. You will perform comprehensive assessments, create care plans, and provide ongoing support to promote patient independence and reduce readmissions.

The role requires RN licensure, knowledge of OASIS, and experience with infusion therapy and home care procedures.

Qualifications

• RN with licensure and OASIS knowledge required.
• Experience with infusion therapy, venipuncture, diabetes management, wound care and related skills.
• Current RN licensure and BLS certification required; BSN preferred.

Responsibilities

• Conducts and submits a complete Comprehensive Assessment per OASIS and agency policies.
• Identify care and support needs and obtain orders to address priorities.
• Make admission decisions based on eligibility, acuity, and safety.
• Obtain consent for care.
• Demonstrates knowledge of payer criteria for skilled needs, intermittent care, homebound status.

As a Nursing Admission Specialist, you will initiate and/or resume home care for patients of all ages and health backgrounds, offering comprehensive assessments, care plans and ongoing support to meet their unique needs. Your efforts help them stay independent and confident when managing their own care, ultimately reducing hospital readmissions and poor outcomes. This position offers rewarding experiences and multiple resources for enhancing your nursing skills, education and professional development. This role will provide support throughout the western region of the Greater Cleveland Area.

A caregiver in this position works Monday through Friday from 8:00am to 5:00pm or 5:00pm to 9:00pm. Requirements include on-call once per week, 1 weekend every 4 weeks and 1 winter/1 summer holidays.

A caregiver who excels in this role will:

• Conducts and submits a complete and accurate Comprehensive Assessment in accordance with OASIS reporting requirements and agency policies and procedures.
• Identifies immediate care and support needs and obtains orders to address psychosocial and treatment priorities.
• Makes appropriate admission decisions based on patient eligibility, acuity and safety.
• Obtains consent for care.
• Demonstrates knowledge of coverage criteria based on payer source, including: skilled needs, intermittent care, homebound status, and physician ordered care.

Minimum qualifications for the ideal future caregiver include:

• Graduate of an accredited school of Professional Nursing
• Knowledge of Home Care and the OASIS data set
• Experience with Infusion therapy, venipuncture, diabetes management, wound/ostomy care, urinary catheterization, enteral feeding, Oxygen Therapy, administration of injectable medication and Left Ventricular Assistive Devices
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Valid Driver's License (VDL) upon hire and per policy 8-175 thereafter
• Proof of automobile $100,000/$3,000,000 insurance coverage and per policy 8-175 thereafter
• One year of current clinical experience as an RN
• One year of experience in an acute medical/surgical or home care setting

Preferred qualifications for the ideal future caregiver include:

• Bachelor's of Science in Nursing (BSN)
• Six months of home health care experience

Physical Requirements

• Must be ambulatory and able to drive to patients homes and potential customers, able to climb stairs, pull, push, and facilitate movement of the patient.
• Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.

Personal Protective Equipment

• Follows standard precautions using personal protective equipment as required.

Pay Range

Minimum Annual Salary: $73,840.00

Maximum Annual Salary: $115,648.00
