AuthoraCare Collective is seeking a Hospice RN Home Care Case Manager for the Greensboro service area. This is a full-time role, 32 hours per week over 4 days, requiring CPR/BLS and NC RN licensure.

You will manage care plans, collaborate with the IDG, and communicate with physicians, pharmacists, and caregivers to ensure quality, compassionate patient care. A strong emphasis on on-call support and patient education is included.

Qualifications

• Graduate of an accredited School of Nursing.
• Current North Carolina RN license in good standing with the state board.
• Experience in acute, hospice, home health, or long-term care is a plus.
• CPR/BLS certification required.

Responsabilités

• Manage the overall plan of care for a caseload of hospice patients in collaboration with the interdisciplinary group.
• Assess and identify patient/caregiver problems, establish nursing goals and interventions.
• Coordinate with physicians to establish the medical plan of care for the patient.
• Establish and maintain therapeutic relationships with patients and caregivers; provide emotional support and teaching.

AuthoraCare Collective is currently seeking a Hospice RN Hospice Home Care Case Manager for our Greensboro service area. This position is full-time (32 hours per week / 4 days). CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Job Summary

Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG) . Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient.

Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

Requirements

Education/Experience:

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• Experience in an acute, hospice, home health setting, or long-term care setting a plus!

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Other:

Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
