Job summary

The VNA of the Treasure Coast is seeking an RN Admissions Nurse to support the Hospice Home team. The role conducts skilled nursing assessments and determines hospice eligibility, coordinating with the Hospice Medical Director, physicians, IDG, and other agencies to optimize patient care and resource use.

Responsibilities

• Identify patient/family needs and provide admission services in accordance with guidelines.
• Provide pre-assessment and admission services in home or facility as needed.
• Coordinate with Hospice team to obtain records and refine the plan of care.
• Complete admission process including education, consents, and EMR documentation.
• Establish initial care plan and medication profile; review with Pharmacist and arrange DME.

Job description

The VNA is seeking an RN Admissions Nurse to work with the Hospice Home team. This position provides skilled nursing assessment and determines eligibility for hospice services.

Identify patient/family needs and provide admission services in accordance with the federal and state guidelines.

Responsibilities:

1. 1. Assumes primary responsibility for initial patient/family education related to hospice election, services, and hospice coverage as it relates to the hospice diagnosis. Initiate communication with Hospice Medical Director, attending Physicians, Interdisciplinary group (IDG) and other agencies, needed to coordinate optimal care and use of resources for the patient/family and their immediate needs.
2. 2. Provides information, pre-assessment, and admission services to patients, conducting pre-assessment in the home or health care facility as necessary.
3. 3. Obtain up-to-date medical records, seek input from other team members regarding the patient plan of care, and obtain additional knowledge that supports hospice eligibility and end of life prognosis
4. 4. Completes the admission process with patients and families including explaining hospice services, insurance benefits, completing all required consents and required documentation in the EMR.
5. 5. Establishes initial POC , medication profile, conducts medication review with Pharmacist and orders an initial DME
6. 6. Keeps Patient Care Manager and Admission Coordinator informed about progress of referral/admission or NTU
7. 7. Inform the RN Patient Care Manager of unusual or potentially problematic patient/family issues.
8. 8. Meet regularly with Hospice nursing staff to review problems or unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.
9. 9. Share in providing 24-hour, seven-day week coverage to patient/families.
10. 10. May be responsible to take after hours or weekend on call.
11. 11. Will work one summer and one winter holiday.
12. 12. Assume role of associate nurse during on-call time or to fill patient/family needs during the absence of other primary nurses.
13. 13. Provide appropriate support at time of death and period of bereavement if indicated.
14. 14. Participate in VNA Hospice orientation and in-service training programs for professional staff.
15. 15. Participate in agency and community programs as requested, to promote professional growth and understanding of Hospice care.
