Job summary

Haven Hospice in Gainesville, FL seeks an RN for Hospice Admissions to visit patients in homes, hospitals, and facilities within the Gainesville coverage area. You will assess eligibility, explain hospice services, obtain consents, and coordinate the start of care with nursing, aides, and equipment, while documenting meticulously.

Qualifications

• Graduate of an accredited nursing program with Florida RN licensure.
• Hospice nursing experience preferred; 1 year minimum; 3+ years in related settings acceptable.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Visit patients (homes, hospitals, nursing facilities) referred for hospice care.
• Explain hospice services, discuss goals and care with patients and families, obtain consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year of hospice nursing experience required, 3+ years preferred; or similar settings such as Long‑Term Care, Intensive Care, or Emergency Room considered in place of hospice nursing experience
• Must be a licensed driver with an automobile insured in accordance with state or organization requirements and in good working order
