Job summary

Suncoast Hospice, a part of Empath Health, seeks an Admissions RN to coordinate patient admissions across facilities in Pinellas and Hillsborough counties, with a Tue-Fri 9a-7p schedule. The role supports end-of-life care, educating families and coordinating care in diverse settings.

The Admissions RN will assess patient needs after terminal diagnosis, provide compassionate care, and work within a mission-driven team to ensure Full Life Care for All.

Qualifications

• Must possess a clear and current Florida Licensure as RN.
• BSN or MSN preferred.
• Minimum one year experience in an acute-care Hospital / In-patient environment providing direct patient care.
• Previous Hospice RN experience preferred.

Responsibilities

• Coordinates patient admissions activities in the long term care facility, patient home, hospital, assisted living facility and hospice house settings.
• Educates patient, family and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, ethical concerns and safety practices.
• Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.
