Job summary

Suncoast Hospice, a part of Empath Health, is seeking a caring Admissions RN to coordinate patient admissions across diverse settings in Pinellas County. The role emphasizes education for patients, families, and caregivers and requires a Florida RN license with hospice experience preferred.

Qualifications

  • Must possess a clear and current Florida RN license.
  • BSN or MSN preferred.
  • Minimum one year experience in an acute-care hospital/in-patient environment; hospice RN experience preferred.

Responsibilities

  • Coordinates patient admissions activities in long term care facilities, patient homes, hospitals, assisted living facilities and hospice houses.
  • Educates patient, family and caregivers regarding disease process, palliative interventions and safety practices.
  • Provides initial assessment of the impact of terminal diagnosis on physical, functional, psychosocial and environmental needs and activities of daily living.
