Job summary

Hospice of Marion County, part of Empath Health, seeks an Admissions Registered Nurse to guide the admission process for Marion County patients. The role emphasizes assessing eligibility, coordinating with referrers and physicians, and ensuring timely access to appropriate care in accordance with care programs.

Schedule: 4 days/week, 11am–9pm or 12pm–10pm. Hospice experience preferred. Requires an Associate degree, Florida RN license, and valid driver’s license with reliable transportation.

Qualifications

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training.
• Licensure: Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
• For field-based positions, reliable transportation is required.

Responsibilities

• Assess eligibility for Empath Health Network of Care and advocate for access to the right care.
• Determine level of care and financial classification for new admissions.
• Collaborate with community referral sources and Empath Health physicians regarding patient eligibility and care plans.
